The CDC changed the definition of "obese". Based on insurance industry records, it seems that people at the heavier end of the previous "normal" spectrum were dying more quickly than people at the lighter end.

Bigger portions also have a pretty direct influence, especially the quart-or-larger-sized soft drinks.
They just aren't the whole story.
